MMU2S Skipped Layers
I am very happy how Prusa Research has fixed the filament loading problem so it can be loaded more reliably.
I'm getting very consistent filament loads and unloads but I'll still occasionally get skipped layers. I find that my MMU loads the filament to the nozzle correctly, trips the IR sensor and continues printing. Occasionally though, the MMU idler motor won't disengage from the filament. This leads to the extruder grinding on the filament and skipped layers result. The MMU still actually unloads the ground filament successfully. The only thing missing from successful tool changes is the MMU barrel not rotating off the filament.
What could the issue causing this be?

Sounds like a firmware bug to me. You may want to report it.
