What causes this?
I got my MK2 recently (pre-assembled) and this sort of thing has happened a few times.. http://imgur.com/a/TDotl
I'm not really sure what happens as I've been out of the room each time it's happened... I can only assume it's partially due to the filament not sticking to the bed enough... is that correct? I've also noticed parts of my prints lifting off the bed, but aside from the auto bed leveling, I haven't done any calibration.

Yes, your filament has come loose from the bed. If you haven't done any tuning at all, then you need to do live-z adjustment.
