What is this problem and how do I fix it?

What is going on here and how do I fix it? Layer shifting is my hunch…I heard the nozzle hitting the piece before it came apart. 

This is a used machine that was advertised as having a layer shift problem but I was able to get it sorted with some tension adjustments on the X belt. It was never this bad. In fact I don't know that I saw any issues. I just adjusted it when I unboxed it and it has been pretty good until now.

I am out of adjustments now…the bolt is completely backed out. I don't know if I need to put it back in for a few turns or what. I find the lack of precision about all of this frustrating. Too many things seem to be about feel or "that's about right" when it would be better to have a measurement to work from. I know there is an official belt tension gauge on Printables.com but it has to be made in PETG: seems to me it could be shipped in the box with the other tools, since not everyone has a spool of PETG. 

Anyway, is this layer shifting? What are the causes? How do I resolve it?

Seems like it's the Z axis not raising the nozzle to clear the surface of the workpiece (I have seen that before on a different machine). I had job reset the machine today, no idea why…just a loud beep and it tried to recover the job. Trouble is, it started extruding material about 1 inch about the previous layer.

It's adhered pretty well. I tightened the idler adjustment all the way in (maybe the belt breaks and I have to redo the whole thing…that's how annoying this is) and ran the job again. It completed without any issues. The print in place movements don't work 100% but I ran the same job on a different machine and it was a lot to fork to free up the parts so I am attributing that to the model.

I was suspecting warping lifting part of the object into the printhead path causing  crash which frequently leads to layer shift.  It's clear there are overhangs, which sometimes warp upwards, but it wasn't clear whether bed adhesion was contributing.

You might try the same mechanism in a dark or clear filament which tend to behave better with fine tolerances - or go back to CAD and ease them.

It's well worth getting a spool of PETG, not just for the belt tension meter but also for parts needing more resilience or more UV resistance.

I am giving up on this model for now. I have a spool of PETG waiting via my PrusaMeter activity so I'll see how it goes whenever I decide on a color.

There actually aren't any overhangs. I did try it one more time and it seems to fail in the same way…the center section (not visible in the image above but see the model) comes loose somehow. Trying a similar one that I have gotten to print successfully.
