printing lithophane - extruder skipping

Hi,
does anybody have experience with printing lithophane from photos? There is an article on Prusa blog, so wanted to try. Without success.
Extruder makes troubles, there is most probably many small print areas, that cause small extraction and retraction very fast. Fillament is grinded off by the extruder pulleys and extruder starts to skip (clicking sound). That is the end point.
I tried to fasten/loosen the springs, slow down the speed (up to 30%), use another fillament. Nothing worked.
I am running MK2.5 with bonthech drive gear. Do you have any tips?

One more point. Prusa blog contains guide telling to generate model as "negative". I tried, but it makes it really negative, so the colors are inverted when the light goes through. Do you know why? Shouldn't it be possitive better?
