Multiple issues Lines, holes and drips!

What is wrong with this print?  Its not really as bad as this photo makes it look, but why am I getting the lines on the top layer?  I'm running on a textured sheet and it hides a lot of the imperfections but how do I tune it out?  Is it too low or not low enough on the first layer?

Next, why am I getting these holes on the layers?  I don't really know where to look to get rid of this one - maybe an under extrusion?

 

 

Third problem (and maybe a clue to the issue - I got 2 burned plastic blobs on this print.  Now normally people say "nozzle nozzle nozzle" but in this case I don't think it's the nozzle simply becasue this is what it looks like after the print.  It looks like it is leaking out of the temperature check hole?

I did get the end covered in filament one night and so it is still fairly dirty - the thing that I believe caused the issues in the first place...

Well - I can't be 100% sure - but it looks like you've changed the nozzle but didn't follow the guide (E3D or Prusa's) to the letter.  Each and every step is vital to avoiding print problems.

The nozzle MUST have a gap between the HEX and the aluminum heater block: no gap ensures a leak and poor performance.

R & R the nozzle, but follow the guides - then retry the print. I suspect most of the issues will be gone. You may have to remove the hotend to gain access to properly set the heat break depth in the heater block, but you might be able to twist the heat sink ... but you've probably got plastic in the threads that needs to come out for get the heat break/nozzle seal right. A full clean is probably warranted.
