Filament IR Sensor reading filament has run out when it has not.

I have been all through the web looking for a reason that this could be happening or a fix.  Recently my prints have been stopping mid print and my MK3S has been prompting me to swap filament as if I had run out...

I checked the sensor to read when it did and didn't prompt me that there was filament inside the extruder and when I "Jimmied" the filament it would flash from 1 to 0 when the filament was leaned to the right side of the entry point into the extruder.

At this point I opened up the extruder and cleared out some debris and dust from the area.  I saw the ball push the the arm into the IR Sensor as it should when filament was inserted into the extruder and figured that it was the debris that caused the problem.  I did notice the arm was not going back into the sensor all the way as if I pushed it back fully with my hand.  After reassembling I was back to the same issue.  I can get it to not give any false alarms if I loosen the extruder tightness but then my prints come out incredibly thin...

I am not sure if I am missing something here, I don't see how this is something that could happen over time... this happend after a full year of print time....

Has anyone experienced this in the past and know a fix?
