What caused this bearing failure

I received my mk2s kit 2 weeks ago, assembled and have printed many things. First, I love this printer - it is so much more user friendly than my delta printer.

Printer Statistics show 4d 18h 1m of printing time. I started getting errors in prints with the Y-axis shifting during a print. First I thought the belt was too loose, but when adjusting it, I noticed the bed doesn't slide smoothly along the Y-axis. Turns out it was a bearing. It is hard to see in the image, but the 1 row of little bearings seem to be wider. The rod already has a couple grooves worn out as a result.

Did I over tighten the bearing to the bed plate? Were the bearings not square when I built it? Or is it just a bad bearing? I'm pretty sure it didn't look like this when I assembled the printer.
