I'll finally be able to get the adjustments done tomorrow. Once I find out what's going on, I'll let you all know about my findings. I'll do my best to get a snap of the pulley as well.

David is right. Most likely the Z frame is not perpendicular to the Y axis and this has to be corrected mechanically. There is no way the mesh bed leveling could detect such a problem.

How on earth did you figure out David was right? It was VortyZA that made the suggestion first! David simply wanted to be annoying and the fact you have not actually read the posts does not help either!
I have no idea why I bother with this forum Prusa do not care about their own support forum and it seriously annoys me! I take it David works for Prusa or has something to do with it as you certainly know how to single out a member!

I had the same issue with my printer. The problem is that a nut and a washer on the threaded rod doesn't give you any ability to adjust the angle of the frame.
In the end I packed out one side of the washers with aluminium foil folded as many times as it took to get the frame perpendicular.
I think the washer should be twice the diameter to be able to pull the frame square from tightening it up alone.
On the upside, after rerunning calibration again I got a "Congratulations, you built your printer precisely," result 😉
