self test fail

My i3 Mk 2.5 is failing on the self test as well as bed leveling.

On the self test it says left side hotend fan wiring error. the fan is running at the time.
On the bed leveling it says the nozzle has debris and crashes into the plate

I have done a full reset as well as tested the fans and sensors with a meter. I can control motors and fans by command line in
pronterface

Is there a possability that the controller board is damaged.

This unit was assembled prior to shipping and has worked great for many hours.

It always used to be that the Mk2 asked the question: "Is the fan running" and you had to select "Yes" from the LCD. If you just clicked the button then it would think you told it the fan wasn't working 🙁

And it's likely that your probe is too high.
