how do i make a smooth top layer without ironing?
not sure what to change or is this normal i would like ti to be smooth without ironing it.

from your picture, my suggestion is increase the perimeter, maybe 3 or 5 perimeter. i think it will make it better

That top infill looks massively over extruded though. I get surfaces much much smoother than that without ironing with my mk3.

I think what could be happening is that it looks over extruded since it doesn't have time to cool. I've messed with some setting when I had a similar problem.
@sandysasmita is on the right track. I got it to look better by removing the need for doing a top infill by increasing the perimeter so it did not do any infill. Or you can try making it 1 or 0 perimeters to give it more room to move back and forth. Both options can be done on just the top layer with changing the Perimeter setting with the Height range Modifier.
Also try slowing down on the top layer, decreasing your temperature, changing the infill angle, and/or increasing your fan speed.

I had a similar model with narrow parts. I was able to get my surface smooth by using Concentric as the top fill pattern.
