More Under Extrusion

I have 2 Prusa Mk3 printers. One prints fine and the other (the oldest one) is experiencing some under extrusion.  I posted about this problem about a week ago and got a couple suggestions and tried them but since then (another 30 printing hours or so later) the problem has gotten worse,  I thoroughly cleaned out the hotend, replaced the nozzle, adjusted the pin on the bondtech gear, replaced the heater cartridge, did a factory reset, lubed up the gears and just gave everything a general good cleaning. I am still experiencing under extrusion. I don't think it's my slicer profile. Something mechanical has to be causing this issue. I use the same profiles for both printers and have 12,000+ printing hours between the 2 of them. Currently, we are going to replace some of the plastic print head parts, the thermistor and give it another good cleaning. This problem started fairly minor, only experiencing problems when printing fast top surface layers but now we are experiencing under extrusion even on the slow first layer. In the video you can hear what sounds like some clicking, it sounds like its the springs for the bondtech gears that are making that noise. That is the same noise I get when adjusting the tension on the gears. Any suggestions would be greatly appreciated!

First layer should be lower, a little bit.

Do several times filament change, to clean the inside, also try it with higher temperature filament.

A big flat top, prusa can not do this very good, slow down or try with higher temperature. Test this with a simple bigger flat stl file, i did this.

Higher temperature.

Slow down.

Different filament.
