Hot end crashing in rear left corner

This is a factory assembled, almost new Mk4S. 

I'm trying to print out a spider with a lot of parts. I tried this a couple of weeks ago, but kept getting "filament stuck" errors, so I set it aside and did a few other things, but now am getting back to it. After unloading and reloading the filament on a few times, I decided to wash the PEI plate before trying again. While washing the plate, I noticed that the head had smashed gouges in the plate in the rear left quadrant. I flipped the plate over to use the other side, but then realized that the last time I'd tried to print it, it had smashed gouges in that side of the plate, too.

This made me think that there was something wrong with the Z mesh bed leveling, but it appears that the Mk4 doesn't have those settings any more. I recalibrated the load cell, but that had no effect. I watch the printer check its bed calibration before printing, so I know it's doing something, but it isn't doing the right thing.

It's a factory assembled unit, so I don't know if there are screws that I should be checking, and I've run out of troubleshooting steps. Any ideas?
