Collision Detected Warning

I've been trying to calibrate my MK4S and make sure everything is working correctly. I've had a problem with layer shifting, but I'm hoping that is now solved with proper belt tensioning and ensuring no wires are hitting each other. 

My problem now is that I'm getting a collision detected warning on the z-axis when there is nothing around the extruder. I've tried recalibrating the z-axis and re-doing the load cell test. The load cell test is a bit flaky. It will start the 5s timer and then end before the timer counts down to zero saying that I've touched the nozzle even though I haven't. Based on this, I'm thinking that perhaps the load cell is faulty or something needs to be cleaned (which is interesting given that I've only printed 2 successful 8x8 multiboard and 4 unsuccessful (layer shifting). Attached is a photo of the nozzle. Any suggestions?
