MK4 with MMU3 in Prusa Enclosure print fail due to layer shift / crash?

Guys, can you give us a hand figuring out what happened here?
MK4 with MMU3 in Prusa Enclosure
MK4 is running 6.0.4+14924
MMU is running 3.0.3
Prusa Slicer 2.0.8
black/red/white prusament pla
enclosure + MMU buffer has these mods:
https://www.printables.com/model/461663-spool-holder-1kg-or-2kg-for-original-prusa-enclosu
https://www.printables.com/model/541601-horizontal-mount-for-mmu3-fits-the-original-prusa
https://www.printables.com/model/751530-mmu3-ptfe-tube-guide/comments
https://www.printables.com/model/845863-mmu3-mounting-kit-for-original-prusa-enclosure

Things I tell at this point:
The "shift" happened on both x and y axis
The "shift" happened on both the print and the purge tower
The print was still carrying on when I discovered the "shift", so I assume crash detection (if it exists for the MK4) didn't pick it up
I unplugged the USB stick from the printer, plugged it into my PC and read it directly off the drive, and have found no issues (gcode viewer didn't show any "shift")
I moved the nozzle to the "shift" Z height, can confirm that it can't hit anything inside the enclosure within its full range of motion (X axis)
The bed can't hit anything inside the enclosure during its full range of motion (Y axis)
On the small pieces that snapped off, there is a blob of hard pla blob (as if the nozzle got caught and have stayed there for a short while)

So guys can you tell what happened? I want to print this model again but avoid this happening again.

Also, what is up with the floating lines right at the end, above the purge tower? (see attached screenshot).

Thanks team.
